The NCQA is a private organization dedicated to improving health care quality.
What is PCMH?
Patient Centered Medical Home is a health care setting that improves partnerships between patients and their personal physician(s), and when appropriate, the patient’s family.
This concept includes:
- accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective care. There are nine standards; including 10 must pass elements, which can result in one of three levels of recognition. The standards are: access and communication, patient tracking and registry functions, care management, patient self-management and support, electronic prescribing, test tracking, referral tracking, performance reporting and improvement, and advanced electronic communication.
